BAHAN DAN METODE: Kelangsungan hidup gigi dalam jangka panjang
Populasi dan rancangan penelitian
Sebanyak 175 pasien yang disertakan dalam basis data milik peneliti yang berisi tempat-tempat yang dirawat dengan GTR dari penelitian-penelitian yang telah dipublikasikan sebelumnya dipilih untuk berpartisipasi dalam penelitian ini dengan mengidentifikasi kasus-kasus yang muncul selama pemeliharaan periodontal dalam periode 4 bulan antara Februari hingga Mei 2002. Semua subyek sebelumnya telah menerima perawatan periodontal komprehensif, termasuk instruksi kebersihan mulut, scaling, prophylaxis, dan terapi GTR pada sekurang-kurangnya satu cacat intra-tulang minimal 2 tahun sebelum penelitian ini. Semua pasien saras sistemik, tidak memakai medikasi, dan punya kebersihan mulut yang baik pada kala bedah periodontal. Root planing semulut dilakukan dengan atau tanpa akses flap bila diperlukan. Pada setiap pasien, satu tempat yang menampilkan cacat intra-tulang diberi perlakuan dengan GTR pakai membran penghalang non-resorbable atau aborbable, seperti yangn dideskripsikan sebelumnya, atau kombinasi membran absorbable dan biomateri aloplastis. Selama tahun pertama setelah selesainya perawatan aktif, semua pasien dikontrol dengan jeda bulanan untuk diberi instruksi ulang dalam prosedur kebersihan mulut dan pembersihan gigi profesional. Hanya satu cacat per pasien disertakan dalam penelitian ini. Dalam kasus-kasus yang menerima perawatan GTR di banyak tempat, cacat pertama yang menerima perawatan dipilih untuk penelitian ini.
Genap 1 tahun, semua pasien diberi kesempatan untuk ikut serta dalam program pemeliharaan periodontal yang meliputi instruksi kebersihan mulut dan pembersihan gigi profesional setiap 3 bulan. Sebanyak 58 pasien tidak ikut dan menerima perawatan berbasis komunitas dari dokter gigi umum mereka dan satu kali kunjungan untuk kebersihan mulut dan pengukuran-pengukuran tindak lanjut di tempat praktek peneliti sekurang-kurangnya setiap tahun kedua.
Pengukuran klinis
Skor plak semulut dan skor plak lokal direkam sebagai persentasi dari total permukaan yang menyingkapkan adanya plak. BOP dinilai secara dikotomi dengan gaya 0.3 N pakai probe peka tekanan manual. Kedua macem skor tersebut lantas dihitung. PD, CAL, dan REC direkam hingga milimeter terdekat. Pengukuran dilakukan pada titik permulaan sebelum perawatan GTR dan setelah genap 1 tahun. Kemudian, CAL dan PD diukur selama kunjungan-kunjungan pemeliharaan, sekurang-kurangnya setiap tahun kedua. Percobaan-percobaan untuk meminimalkan bisa potensial yang berasal dari fakta bahwa adalah klinisi yang sama yang melakukan perawatan, termasuk membuat rekam medik pasien tidak tersedia sebelum semua pengukuran tindak lanjut. Kedalaman sudut radiografi cacat juga diukur di titik waktu permulaan pada radiografi periapikal seperti yang telah dideskripsikan sebelumnya.
Evaluasi Karakteristik Pasien
Setiap pasien selanjutnya dikarakterisasi sehubungan dengan deskriptor tambahan yang dikenal penting guna menentukan prognosis terapi periodontal konvensional, yaitu: umur, macem kelamin, status merokok, dan partisipasi dalam program pemeliharaan. Perokok didefinisikan sebagai orang yang merokok ≥ cigarette per hari. Keikutsertaan dalam program pemeliharaan didefinisikan sebagai kepatuhan pada kontrol setiap 3 bulan selama periode penelitian (sampai 16 tahun). Pasien yang tidak ikut serta dalam program ini menerima perawatan berbasis komunitas dari dokter gigi umum mereka. Suatu subset yang berisi 86 subyek yang dipilih secara acak dievaluasi lebih lanjut sehubungan dengan polimorfisme yang telah dilaporkan sebelumnya pada komplex gen IL-1 pakai layanan penentuan genotip (genotyping service) yang tersedia komersil.
Analisis Data
Analisis statistik dilakukan pakai paket perangkat lunak statistik. Analisis kelangsungan hidup dilakukan pakai pendekatan multivariate dengan definisi kejadian sebagai berikut:
- gigi tanggal;
- penyusutan ≥2 mm CAL sehubungan dengan CAL pra-GTR di titik permulaan;
- penyusutan ≥2 mm CAL sehubungan dengan CAL 1 tahun pasca GTR.
Metode Kaplan-Meier dipakai untuk mengkonstruksi plot kelangsungan hidup dan analisis life-table. Signifikansi dampak karakteristik pasien pada kelangsungan hidup gigi yang diberi perawatan GTR dinilai dengan mengkonstruksi model ancaman proporsional multi-variate Cox.
